Registration

Eve= ry company must register with the Department of Small and Local Business Development (DSLBD). Your information is kept confidential and is used for tracking CBE compliance as required by local government. To register, compl= ete the form and select Submit. Fields marked with an asterisk are required fie= lds. You should monitor the email address you supply in the Public Contact Information section for notices and other information from DSLBD. If you are applying to become a certified CBE company, you will use the username and password created here to apply for certification online. If your company is awarded a contract that requires you to submit a subcontracting plan, you w= ill use the username and password created here to login and enter payment information into the CBE compliance system. If your company is a CBE on a subcontracting plan, you will use the username and password created here to verify that you have received payments from the prime contractor on a contr= act. All information is kept on a secured server and is accessible only by you a= nd DSLBD.

Company Information section

Provide the following information:

·            = ;   Company Name – The full name of your company

·            = ;   FEIN/SSN Number – This is the tax identification number you put on your company's income tax forms it may be = your Social Security Number. This 9-digit number is generally issued by the IRS.=

·            = ;   Business Phone – The phone number for = your company

·            = ;   Website – Provide the URL for y= our company's website if applicable

Public Contact Information section

Provide the name of a person in your company who will be available in case a question arises. This information will be displayed on = the public website:

·            = ;   First Name

·            = ;   Last Name

·            = ;   Phone – Enter a number where = this person can be reached

·            = ;   Email – Enter an email address where DSLBD can send messages, this should be the one that is used frequent= ly

·            = ;   Username – Enter a username that= you will easily remember.

·            = ;   Password – Enter a password that= you will easily remember, the password must be between 5 and 25 characters and = is case sensitive

·            = ;   Confirm Password – Enter the same passwo= rd that you entered in the Password field

Business Location Address section

Provide the Bus= iness Location Address of your company. According to the DC Law, if you are a= pplying to become a CBE, your company’s principal office must be physically located in DC.

Mailing Address section

Provide the Mai= ling Address of your company if it is different from the Business Location Address. If the address is the same, select the checkbox to pre-populate address information.
